Case Managers are essential to the financial stability within a hospital.
Case Managers employed with Vista Health System work closely with the health care team to assess, plan, implement, monitor, and evaluate options and services to facilitate the progression of patients across the care continuum.
Overseeing management of a patient from the moment they enter the hospital to the moment they leave is one of the primary duties of a case manager.
A Case Manager can effectively balance both the clinical and financial outcome of each individual.
Striving to provide the best possible care, a Case Manager can synchronize the goals of the patient along with the clinical goals of the hospital.
JOB REQUIREMENTS Bachelors degree in Nursing RN license in the state of IL.
At least 2 years of recent Case Management nursing experience in a hospital setting Excellent communication and negotiation skills Ability to manage conflict Proficient in the use of computers Excellent time management skills and ability to set priorities.
Ability to work with physicians, insurance contacts and various outside agencies with a high degree of courtesy, care, and respect We offer a competitive benefit package of Medical, Dental and Vision plans.
