On-call Crisis Worker

General DescriptionThe On-call Crisis Worker/Contractor is not an employee of LCHD/CHC butisretained on an independent contractorbasis, and is not entitled, nor will be entitled to any LCHD/CHC benefits, liability insurance coverage, or workmen's compensation coverage.
All insurance coverage is at the Contractor's expense.
The Contractor will be paid ($175.
00) per mental health crisis assessment.
The Contractor is responsible for all payments and accounting relating to Contractor's federal and state income taxes, Contractor will receive a 1099 from LCHD/CHC.
The Contractor is responsible to maintain all qualifications including certification on the Childhood Severity of Psychiatric Index (CSPI) and state licensure, if applicable.
On-call Crisis Worker Key Responsibilities Take on-call shifts as assigned by CABS Program Coordinator or Assistant Clinical Coordinator(s).
If unable to complete the assigned shift, the Contractor must notify Program Coordinator or Assistant Clinical Coordinator(s) within 24 hours of the scheduled shift or as soon as possible.
Provide all on-call Screening, Assessment, and Support Services (SASS) and related functions in accordance with the SASS and Managed Care Organization (MCO) contracts.
Coordinate linkage and referral services as required by SASS/MCO contracts.
Complete all appropriate written documentation for the SASS crisis assessment, consent forms, and associated documents, and submit documentation to Program Coordinator and Assistant Clinical Coordinator(s) within 24 hours of the conclusion of the assessment.
Maintain client confidentiality and adhere to HIPPA and HITECH standards, signed Business Associate Agreement and Non-Disclosure Agreement.
Conduct oneself in a professional and ethical manner when representing the CABS program and LCHD/CHC.
Attend all required NextGen training and put necessary and appropriate data into NextGen within 24 hours.
Use LCHD/CHC-owned computer equipment for NextGen entry and be responsible for the safekeeping of said computer equipment.
Essential Functions Bachelor's degree in related human service field.
Driver's license in good standing with auto insurance.
Mobile on-call responsibilities arerequired and could be scheduled any day of the week including weekends and holidays.
Will come into the office as needed upon request of the manager.
Supplemental InformationAt this time, you must reside in either Illinois or Wisconsin to work for the Lake County Health Department.

The check will bounce and the victim is left responsible.
